Post-dialysis blood urea nitrogen value prediction using machine learning

چکیده

【目的】透析前に透析量を予測することはいまだ困難である.本研究では透析前のデータから透析後BUN値を予測する機械学習モデルを開発し,その精度と有用性を評価した.【方法】当施設における2020年5月~2022年5月に血液透析を受けた患者の採血,患者特徴,透析データを収集した.機械学習モデルはXGBoostを用いてランダムに分割された全体の70%の患者で学習し,残り30%の患者でテストした.さらに,shapley additive explanationsを利用し特徴量重要度を計算した.【結果】患者246人の935透析データを対象とした.機械学習モデルの精度はR2(決定係数)=0.87であった.予測に最も影響を与える上位3変数は透析前BUN値,透析前体重,血流量であった.【結論】本研究で開発した機械学習モデルは透析後BUN値を高精度に予測でき,その値からKt/Vの推定に応用できる可能性がある.
